Why Malay is hard to understand
Unreleased final stops
- Word-final consonants close without a visible burst
- Standard Malay word-final plosives /p/, /t/, and /k/ are typically unreleased — the closure is held without the air burst. On a video call, these final consonants produce no visible lip or jaw movement at closure. A deaf caller sees the vowel movement and then silence — the distinction between words that differ only in final consonant is phonetically real but visually absent.
Malay-English code-switching
- Educated urban speakers pivot between two phonological systems
- Educated urban Malay speakers in Kuala Lumpur and Singapore code-switch densely between Malay and English within a single conversation. When the code switches, the phonological system resets: English vowels, English consonant clusters, and English prosody all arrive without visual warning. A deaf caller tracking Malay lip shapes is suddenly reading English ones.
Regional dialect gap
- Kelantanese and Terengganu dialects sound very different from KL standard
- Peninsular Malay dialects — Kelantanese, Terengganu, Kedah — differ sharply from Standard Malaysian and from Sarawak Malay. Kelantanese Malay sounds so different from Kuala Lumpur Malay that even hearing speakers sometimes struggle to follow it. For a deaf caller on a video call with a relative from Kota Bharu, the phonological landscape is effectively foreign.

- In Malaysian Chinese-Malay communities, conversations can switch between Malay, English, and Mandarin — or a Chinese dialect like Hokkien or Cantonese. For a deaf Malay-speaking caller, Mandarin's tonal phonology is incompatible with any Malay-based lipreading strategy, and the switch happens without visual signal.

The general population prevalence of hearing loss in Malaysia was estimated at 21.5% among adults aged 18 and older in national health survey data, with self-reported hearing loss reaching 36.9% in elderly populations using pure-tone audiometry. Among older adults aged 60 and over specifically, 6.4% reported hearing disability in the National Health Morbidity Survey 2018. These figures indicate that hearing loss is not a marginal condition in Malaysia — it affects a substantial proportion of the adult population, with particular concentration among older Malay, Chinese, and Indian Malaysian communities.
Access to audiological services has remained limited despite the scale of the burden. Audiology services in Malaysia were established in the 1960s through American Peace Corps and British Voluntary Service contributions, and have developed slowly since. A 2023 scoping review documented an urgent need to identify barriers to access, improve uptake, and evaluate the benefits of adult hearing services — language that reflects how far provision still lags behind need. Hearing aid costs remain a barrier for lower-income Malaysians, and specialist services are concentrated in Kuala Lumpur, Penang, and Johor Bahru, leaving rural Malay communities in Kelantan, Terengganu, and East Malaysia with limited options.
