Why Persian is hard to understand
Uvular consonants
- Persian uvular sounds are produced at the throat — completely invisible at the lips
- Persian has uvular consonants /q/ and /ɢ/ produced deep in the throat, far behind the lips. The lip shape for /q/ is indistinguishable from /k/ or /ɡ/ — a deaf lip-reader cannot tell them apart. These sounds occur in common Persian words and collapse into the same visual signal as other stops.
Farsi–Dari–Tajik vowel divergence
- Iranian Farsi, Afghan Dari, and Tajik Persian use different vowel systems
- Standard varieties of Persian differ significantly in their vowel inventories — anywhere from 6 to 8 vowel distinctions depending on the variety, with similar vowels pronounced differently between Iranian Farsi, Dari, and Tajik. A deaf person who lip-reads one standard finds that a hearing family member from another community produces vowels in different mouth positions.
Tehran vs. regional dialect
- Isfahani, Shirazi, and Tabriz Persian use different phonological patterns
- Standard Persian is based on Tehran speech, but Isfahani, Shirazi, and Azeri-influenced Tabriz Persian each differ in vowel qualities and prosodic patterns. A deaf person who lip-reads Tehran-standard Farsi finds that a hearing family member from a different Iranian city speaks with mouth shapes that don't map to what they expect.
Diaspora code-switching
- Iranian diaspora families switch between Farsi and English with no visual warning
- In diaspora communities in the US, UK, Canada, and Germany, hearing family members freely switch between Farsi and English within sentences. For a deaf lip-reader, sudden English words produced with Persian phonological influence — or vice versa — add an invisible layer of ambiguity that cannot be resolved without hearing.
Consonant cluster loanwords at speed
- French and English loanwords bring unfamiliar cluster patterns into everyday Persian
- Modern spoken Farsi has absorbed many French and English loanwords with consonant cluster structures unusual in native Persian. In fast speech, these clusters appear in phonological positions that don't match the learned patterns of a lip-reader trained on traditional Persian vocabulary — the mouth shapes don't fit any familiar Persian word.

A population-based study in Tehran found a hearing impairment prevalence of 14.27%, with 5.19% of participants having disabling hearing impairment and 0.48% classified as deaf (PMC5632326). All grades of hearing impairment increased significantly with age, and researchers noted that the considerable prevalence of hearing impairment in Iran — relative to other developing countries — is concerning given the country's demographic trend toward an older population. School-age data from northern Iran found that disabling hearing impairment affected approximately 0.1% of children using WHO criteria, with sensorineural causes predominating.
Iran's national programme for ear health and hearing is expanding its neonatal screening coverage, with the Ministry of Health targeting 100% neonate screening and increased coverage for children aged 3–6. These are meaningful steps for early identification — but they leave the large majority of people with age-related or acquired hearing loss without systematic support. Persian Sign Language (PSL) has a community of users in Iran, but no formal legal recognition ensures daily interpreter access in social and family settings. For a deaf or hard of hearing person in an Iranian household, communication with hearing family members at home remains entirely unstructured.
The GBD 2019 hearing loss study estimated that across the Eastern Mediterranean region — which includes Iran — years lived with disability due to hearing conditions are rising (PMC6796666). As Iran's population ages and the number of people with age-related hearing loss grows, the communication gap at family tables will deepen without technology that bridges it.
